Your participation in Forever Friends helps insure that all children will be admitted for needed medical treatment, regardless of their ability to pay.
As partners in our shared mission to provide access to quality healthcare to the Tri-State Area, your legacy through estate planning can establish a sound financial base for Hoops Family Children’s Hospital, the Edwards Comprehensive Cancer Center, or another Cabell Huntington Hospital department or service.
How to Become a Member of Forever Friends:
Becoming a member is simple! You can call or write Cabell Huntington Hospital Foundation to tell them you have included the foundation as a beneficiary in your estate plan. When you call or write, please indicate the following:
_____a) I am considering becoming a member of Forever Friends.
_____b) I have already placed Cabell Huntington Hospital Foundation as a beneficiary in my estate plan.
_____c) I would like copies of the Estate Planning Guide, Estate Inventory Form, and “My Final Wishes” booklet.
_____d) I would like someone to visit with about estate planning.
A staff member will be happy to help you!
Forever Friends members will be remembered through the inclusion of their names on a plaque that hangs in the hospital’s reception area. In addition, you will receive a tasteful, framed certificate, and an invitation to our annual luncheon.
